Community Health Worker Certificate of Achievement The Community Health Worker Program trains and certifies students to work in the fields of public health, health care, and social services.Community Health Workers provide a wide range of services, including health outreach and education, client-centered counseling and case management, and client and community-based advocacy. Theywork to reduce unequal rates of illness and death between different communities and to promote health equity or justice, prioritizing communities with the greatest health risks. Learning Outcomes 1. Identify basic health needs and evaluate health and human service resources within the community. 2. Demonstrate Community Health Worker Professional skills and jobreadiness in the core competencies identified by the Community Health Care Worker Core Consensus Project (C3) and the needs of the local community.
